What is the age range of our patients?
- We accept patients between the ages of 18 to 60. If you are over the age of 60 you need to see a Board Certified Geriatrician. Please contact Dr. Ayati's Geriatric Concierge Clinic

How much does the membership cost?

What does my membership provide?
- Access to Dr. Ayati via office visits, telemedicine, text, email, phone
- Initial health assessment
- Comprehensive annual physical exam and ongoing access to medical care
- Travel medicine (preparing you for your business or pleasure trip in order to minimize your risk of illness)
- Coordination of care should you need to see specialists, or require laboratory, radiological, and/or physical therapy services
- You can schedule a same day/next day appointment with Dr. Ayati (or his covering physician/ nurse practitioner (NP)) Monday through Friday from 8:30 AM to 5:00 PM.
- Access to Dr. Ayati (or his covering physician/NP) by phone/text from 8:00 AM to 11:00 PM including the weekends and holidays.
- Access to Dr. Ayati (or his covering physician/NP) through electronic visits (i.e : FaceTime, Zoom or Skype) from 8:00 AM to 11:00 PM.
- Personalized assistance with appointment scheduling and reminders for your appointments with the Concierge clinic.
- Should you need to be hospitalized at Stanford, Dr. Ayati will coordinate your care with the Stanford hospital staff and other specialists.
- Invitations to health-related presentations/talks by Dr. Ayati
- Wellness and healthy aging pamphlets, booklets, and scientific articles

Do Medical Insurances cover my membership?
- No. Dr. Ayati does not accept any insurance and insurances do not cover the concierge membership fees.

Do I need to pay any extra fees for laboratories, imaging, prescription drugs, hospitalization, or specialist visits?
- The only fee you pay to our office is your monthly membership fees. For all other services, Dr. Ayati will refer you to centers/specialists who do accept your medical insurances. Therefore, you need to have a medical insurance. All your prescriptions will also be forwarded to pharmacies which accept your regular medical insurance.

How do I sign up?
- Please contact our office and we will set up an initial consultation with Dr. Ayati for you. If Dr. Ayati can accept you and you like to join we will send you the Agreement. As simple as that.
